Sign and date the form, and fax or e-mail it to us along with a copy of your Texas General Lines License and E&O declarations page. We will then contact you directly regarding the appointment.
Requirements- Complete signed and dated Agent Profile Form
- Current Texas General Lines License
- Current Errors and Omissions coverage for minimum limit of $500,000
- One year agency existence
- Agent’s personal credit in compliance with Capstone’s guidelines
- Minimum Annual Production
